I acknowledge that the personal data provided in this personal questionnaire is processed by Orange Slovensko, a.s, No.: 35 697 270, registered in the Commercial Register of the City Court Bratislava III, Section Sa, Insert No.: 1142/B (hereinafter referred to as „Orange Slovensko“), in accordance with Regulation (EU) 2016/679 of the European Parliament and of the Council on the protection of natural persons with regard to the processing of personal data and on the free movement of such data and repealing Directive 95/46/EC, as amended (hereinafter referred to as „the Regulation“), pursuant to Article 6(1)(a) of the Regulation. 1(b) of the Regulation for the purposes of establishing pre-contractual relations relating to the establishment of an employment relationship, including ensuring the relevant legal reporting obligations of Orange Slovensko, a.s. as a future employer to public administration authorities in the field of social security (Social Insurance Institution, relevant Health Insurance Institution, Labour Office, etc.) and, in the case of conclusion of an employment contract, also for the purposes of its performance. The period of processing of the aforementioned personal data is until the termination of the employment relationship with Orange Slovensko, whereby if the employment relationship does not arise at the latest at the time specified in the Acceptance Letter, or at another time agreed between the data subject and Orange Slovensko, the aforementioned personal data of the data subject will be disposed of in accordance with the applicable legislation. I acknowledge that after the employment relationship has been established, my personal data in the scope of personal employee number, first name, surname, maiden name, date of commencement and date of termination of the employment relationship, date of birth, gender, salary data and job position according to the Orange SA catalogue will be provided by Orange Slovensko to Orange SA, which is in the position of a controlling person in relation to Orange Slovensko, on the basis of the legitimate interests pursuant to Art. 6(1)(f) of the Regulation, pursued by Orange SA and Orange Slovensko in accordance with the Regulation, for the purposes of personnel and cost statistics of Orange Group employees. The period of processing of the aforementioned personal data is until the termination of employment with Orange Slovensko.
